In Pakistani conditions, a radiator flush is recommended every:

30,000 km o r 12–18 months
Especially if:

Plain water was used in the past

The van is used commercially o r in hot regions

Flush with a radiator cleaner and refill with 50/50 coolant-water mix to prevent corrosion and scaling.
