Why does my Hyundai Tucson hesitate o r jerk during gear changes?

At times, especially in stop-and-go traffic, my Tucson hesitates o r jerks when shifting gears. What transmission o r performance problems might cause this behavior in Pakistan?

Hesitation o r jerking during shifts can be caused by:
