Why does my Suzuki Swift fail the vehicle emission test in Pakistan despite regular maintenance?

Even though I regularly service my Suzuki Swift, it recently failed the government vehicle emission test. This seems to happen to many Swift owners in Pakistan. What could be causing high emissions despite oil changes and tune-ups? Could it be related to the catalytic converter, oxygen sensor, o r fuel mixture?
