Why is my Suzuki Swift’s transmission not shifting properly?

When I try to shift gears, my Suzuki Swift sometimes refuses to shift properly, and the transmission gets stuck in one gear, either too high o r too low. This issue happens frequently during acceleration o r deceleration. Could this be a sign of transmission failure, and what are the possible causes of this problem?
