Why is the audio output weak o r distorted in my Suzuki Every?

Even after replacing the head unit, some Suzuki Every owners report weak o r distorted sound. Could it be an issue with the speakers o r wiring, and how can it be diagnosed and fixed?

Weak o r distorted audio, even after upgrading the stereo, can be caused by:
